After manufacturing and fitting, the prosthesis is fixed to the abutment, and the point where the screw enters the crown is sealed.This is the key difference between the Multi-Unit system and cemented abutments. All adhesives and cementitious compositions are used outside the patient’s oral cavity, which is beneficial because cement residues can cause irritation and inflammation of the gingiva. Although the cement residue at the junction of the crown and abutment is removed, the dentist can’t always reach tricky areas between the teeth, especially in splinted designs with complex configurations. For single implants, cemented crown fixation is still used.
- Permanent Abutment Fixation: The abutment is screwed in once and for all. Crowns and bridges can be changed from temporary to permanent or from a paired implant to a bridge structure for three or four teeth if additional teeth are lost over time. Throughout these manipulations, the implant remains completely protected and immobilized.
- Better Lateral Load Distribution: Compared to cement fixation, prosthetics on the Multi-Unit platform load the top of the implant less, as proven by studies. This is especially important if implants with a diameter of less than 4 mm are used. These models have a relatively thin wall in the coronal part, and there is a high risk of implant failure if the Ti-base platform is used. Additionally, the Multi-Unit system abutment not only distributes the load better but also acts as a mechanical safety guard. In the event of extreme stress, the abutment screw will break first, not the implant. Removing the broken screw is relatively easy, after which the restoration can be repaired. To prevent recurrence, it is recommended to adjust the problematic crown out of the bite.
- Reduced Micromovement at the Implant/Abutment Level: In Multi-Unit systems, micromovement is significantly lower than when using cement-retained crowns. This is crucial because there is always a micro-gap between the implant and the abutment. If the connection is movable, a “pump effect” occurs—pumping fluid with bacteria through the gingival cuff into the internal structures of soft tissues. If microflora from the oral cavity penetrates into the sterile environment of soft tissues and alveolar bone, it can cause inflammation, potentially resulting in the loss of the implant.
- Easier Achievement of Passive Fit: With screw fixation, achieving a passive fit (no internal stresses) is much simpler. This reduces the risk of loosening and breaking of the screws. The prosthesis on the Multi-Unit can be removed, adjusted, or replaced without internal stresses. Even if local overstresses remain, they are transferred to the area between the abutment and the removable sleeve, where they are easier to manage. Any micromotion that remains is located above the level of soft tissues and will not cause the pump effect or gum inflammation. All operations are straightforward and can be performed quickly and almost painlessly. In contrast, removing cemented restorations is difficult and very traumatic, which is harmful to both soft tissues and bone, as the implant can move, causing additional bone injury.
- Ideal for Complex Cases: Screw fixation of crowns works well in challenging situations with bone deficiency and the inability to line up implants along the dentition. The Multi-Unit system allows implants to be placed at different angles and even in a staggered pattern. Placing the implants at angles to each other is beneficial and recommended, as it allows for even load distribution and reduces the risk of loosening and breaking of the screws. With the Multi-Unit system, achieving a high-quality restoration of the dentition is easier (see the photo below).
- Suitable for Low Crown Heights: If the crown height is 3–4 mm, you also need to buy Multi-Unit screw-retained abutments. Cementation will not hold such a crown in the long term, and the risk of crown loss is very high.
- Easy Removal of Crowns and Bridges: When using Multi-Unit screw abutments, it is necessary to drill through the crown to reach the screw and remove the bridge structure from the abutment. With the cement fixation technique, the crown has to be physically dislodged from the abutment—a traumatic procedure with a real risk of bone damage. The impact is transmitted to the jawbone like a lever.
- Economic and Reputational Benefits for Dentists: Despite the components of the Multi-Unit system being noticeably more expensive, their use is beneficial in the long run. Complex structures on the Ti-base platform often have internal overstresses, contributing to the unscrewing of screws or the destruction of the cement layer (crowns become separated). Such problems frequently occur within a few months after the restoration is installed. There are two possible scenarios:
- The patient goes to another specialist who remakes the prosthesis. The new dentist often switches from Ti-base to Multi-Unit; the prosthesis works better, the patient is satisfied, and remains a client of the new specialist.
- You have to redo the restoration and fix problems at your own expense. These problems arise in difficult cases, and the amount of work is substantial. In any case, this negatively affects your income, although your reputation can be maintained.
